Once you have decided to call me, please leave your name, phone and the best times for me to return your call.  We will have a 10-15 minute telephone discussion to briefly talk about what your issues of concern are.

Patient Forms
Please download the patient forms below.  Print and fill them out and bring these forms with you to your first appointment.  If you are unable to access and print the forms, please call my office and let me know and other arrangements will be made for you.  Please carefully read my practice policies and sign them.  We can discuss any questions at your
first visit.

The First Visit
Please have a seat in the waiting room and I will come out to greet you.  During our first session we will review the information you have shared and explore more in depth what you would like to work on.  First sessions last 50-60 minutes.  I will do my best to make you comfortable.

Fees
If you do not have insurance or do not wish to use your health insurance, I can still help you.  Please see FAQ (frequently asked questions) regarding using your health insurance and for reasons you may not wish to use your health insurance.

For self pay clients, not using any insurance:

First session:  60 minute evaluation, $150
Subsequent sessions:  50 minutes $95

Marital or couples counseling sessions: $125

Payment is due at the time of service.  I accept cash or check. I do not accept credit or debit cards.

Also please carefully read my practice policies so that you understand and agree to follow my policy regarding cancellations and rescheduling.

Contact in Between Sessions
For established clients, I am on call.  For non-urgent matters, your call will be returned by the next business day.  For urgent issues, you may call my cell phone.  You are also welcome to email me with non-urgent questions.  Please be aware that email is often not a secure or confidential medium to communicate.  All appointments and cancellations must be made via telephone.
